MSH offers a comprehensive range of dementia services designed to provide the best possible care and support for those in need. This page provides all the information you need to make a referral into our dementia services.

How to Refer Someone to Our Memory Services

Referring a patient or client to MSH dementia services is straightforward. We accept referrals through multiple channels to ensure the process is as convenient as possible for you.

What Information is Needed for a Referral?

To ensure we can provide the most appropriate support, please include the following information when making a referral:

  • Person’s Full Name
  • Date of Birth
  • Diagnosis Details (including date of diagnosis if available)
  • Contact Information (phone number, address, email)
  • Any Known Care or Support Needs
  • GP or Primary Care Provider Information
  • Emergency Contact Details
  • Any Additional Notes (e.g., language needs, communication preferences, known allergies)

What Support Can Be Accessed?

Memory Support Hertfordshire offers a wide range of services designed to support individuals living with dementia and their carers throughout their journey. Below are some of the key information for the following services available:

1. Dementia Care and Support Services:

  • Day Care Services: Dementia-specific day care centres providing stimulating activities and social engagement in a safe environment.
  • Respite Care: Temporary care solutions allowing carers to take a break while ensuring the individual with dementia is well cared for.
  • Dementia Advisors: Personalised advice and guidance on managing dementia, available through regular appointments or as needed.

2. Emotional and Mental Health Support:

  • Counselling Services: One-on-one emotional support for individuals with dementia and their families.
  • Support Groups: Local and virtual support groups for those living with dementia and their carers to share experiences and find community support.
  • Specialist Dementia Nursing: Access to Admiral Nurses through our partnership with Carers in Hertfordshire, providing expert advice and support for complex dementia cases.

3. Future Planning and Legal Support:

  • Information and Advice: Assistance with setting up powers of attorney, managing finances, and planning for long-term care.
  • End-of-Life Planning: Compassionate guidance on making informed decisions about end-of-life care, respecting the individual’s wishes and ensuring comfort.
